Privacy Policy
Effective date: September 13, 2026
JC Family Dental LLC ("we," "us," "our") operates this website (jcfamilydental.com) for patients and prospective patients of Dr. Jay Patel, DMD. This Privacy Policy explains what information this website collects and how we use it. It does not cover the clinical dental/health records we keep as your treating dentist — those are governed by our Notice of Privacy Practices, as required under HIPAA.
Information We Collect
The only personal information this website actively collects is what you choose to submit through our online Appointment Request form: your full name, phone number, email address (if provided), the reason for your visit, and your dental insurance provider (if provided). We do not use tracking cookies, advertising pixels, or third-party analytics on this site as of the effective date above.
How We Use Your Information
We use the information you submit solely to contact you, confirm your identity, and schedule or discuss your dental appointment. We do not sell your information, and we do not share it with third parties for their own marketing purposes.
Who We Share It With
Appointment request submissions are processed through our website hosting provider's form service (Netlify) solely to deliver your request to our office by email. We do not otherwise disclose the information you submit through this site except as necessary to respond to your request, or as required by law.
Data Retention
Appointment request submissions are kept only as long as needed to schedule and follow up on your visit. Once you become a patient, your clinical dental records (separate from this website) are retained in accordance with New Jersey Board of Dentistry rules — generally at least seven years from the date of your last visit, or, for patients who were minors, until age 23 or seven years from the last visit, whichever is longer (N.J.A.C. 13:30-8.7).
Security
We take reasonable steps to protect the information submitted through this website. In the unlikely event of a breach involving computerized personal information as defined by the New Jersey Identity Theft Prevention Act (N.J.S.A. 56:8-161 et seq.) — such as a Social Security number, driver's license number, or financial account number — we will notify affected individuals in the most expedient time possible, consistent with that law and any law enforcement needs.
Your Rights & the New Jersey Data Privacy Act
The New Jersey Data Privacy Act (N.J.S.A. 56:8-166.4 et seq.) gives New Jersey residents rights to access, correct, delete, and obtain a copy of their personal data, among others. That law's specific compliance obligations apply to businesses that control or process the personal data of 100,000 or more New Jersey consumers annually (or 25,000 or more if the business sells personal data) — thresholds this practice's website does not meet. As a courtesy, and regardless of those thresholds, we will honor reasonable requests to access, correct, or delete information you've submitted through this site — contact us using the information below.
Children's Privacy
This website is intended for use by adults submitting requests on their own behalf or on behalf of a minor patient (e.g., a parent or guardian). We do not knowingly collect information directly from children through this site.
Changes to This Policy
We may update this Privacy Policy from time to time. The effective date at the top of this page will reflect the date of the most recent revision.
